These are the correct translations, using the genitive case to write the dates (months are not capitalized and you literally write “of the month”.)

сентябрь —> сентября
октябрь —> октября

So you would write 4 October 1957 as 4 октября 1957 г. (the г. at the end is an abbreviation of года, meaning year.) This literally means the 4th of October (so this is why the genitive is used) in the 1957th year (no -th suffix needed for the year).

Or 26 December 1991 as 26 декабря 1991 г. Please fix.
